It's quick and easy. Simply
open the appropriate form, and type in your information in the fields provided
(be sure to complete both pages, and add your digital signature). If
you can't or don't wish to add your digital signature, you may leave the form
unsigned, and complete the process when you come for your visit.
Save the PDF to your desktop, then
click the button on the form that says "Click Here to Submit Your Form Electronically" (requires
Adobe Acrobat). Follow
the steps to email your completed patient form to us, and that's it!!
If you'd prefer to fill out your form by hand, you
